The United States is the world's apex destination for clinical complexity — unrivalled in oncology, cardiac surgery, neurosurgery, and fertility technology — but also the world's most expensive, making cost savings irrelevant to every patient who chooses it. Foreign patients come not to save money but because the treatment they need exists nowhere else.

No country scores more extreme on the medical tourism matrix than the United States. Across every dimension of clinical quality — oncologic surgery, cardiac surgery, neurosurgery, advanced imaging, stem cell transplantation, specialized diagnostics, pathology, fertility technology — it earns the highest possible marks. Across every cost-savings dimension — surgical, dental, cosmetic, cancer, IVF, diagnostic, pharmaceutical, mental health — it scores at the absolute floor. These are not contradictions. They are the same fact stated twice: the United States is the most clinically capable and the most expensive healthcare system on earth, and both things drive the approximately 1.9 million inbound medical tourists it receives each year.

The clinical case for the United States

The country operates 73 NCI-Designated Cancer Centers, nearly 800 participating heart hospitals, approximately 3,692 board-certified neurosurgeons, and close to 1,000 MBSAQIP-accredited bariatric surgery centers. Its stem cell transplantation network — with 319 FACT-accredited facilities — is the largest in the world. Advanced imaging infrastructure is unmatched: every major modality is available at academic centers with the highest-generation equipment, CAP accrediting over 8,000 laboratories, and 41 pathology institutions holding international recognition. For a patient with a rare hematologic malignancy, a complex brain tumor, or a cardiac defect requiring multi-disciplinary reconstruction, Mayo Clinic, Cleveland Clinic, MD Anderson, Johns Hopkins, and Massachusetts General are not merely options — they are often the only realistic ones. Cleveland Clinic alone reports that 90% of its foreign patients arrive carrying insurance, underlining that cost is not their calculation.

Clinical trial access reinforces this gravitational pull. The United States hosts over 500,000 registered clinical studies and maintains a 99% expanded access approval rate, giving international patients a route to investigational therapies unavailable anywhere else. For oncology patients who have exhausted standard lines of treatment, this is frequently the decisive factor.

Fertility and reproductive medicine represent another axis of global dominance. The country maintains the world's largest egg donor pool (nearly 20,000 donors nationally), time-lapse embryo monitoring, comprehensive PGT capabilities across all three testing types, and 456 or more ART-reporting clinics — with IVF live birth rates of around 49.7% for women under 35. Patients from the Middle East, China, and Latin America increasingly choose the United States for surrogacy and egg donation despite costs of $150,000–$220,000 for a surrogacy arrangement, because the legal framework (commercially permissive in 48 of 50 states), the donor pool depth, and the laboratory standards are unmatched globally.

The cost reality

There is no ambiguity here. The United States is the benchmark against which every other destination is measured for cost savings — and it is the baseline from which patients flee. A CT scan that costs $1,200 in the United States costs $200–$400 in Thailand. A hip replacement priced at $40,000 in an American hospital runs $12,000–$15,000 in Mexico. Dental, cosmetic, hair restoration, pharmaceutical, and addiction treatment costs are all materially lower at competing destinations. Any patient whose primary motivation is affordability should look elsewhere.

Where the picture is more nuanced

Some factors sit in the middle of the scale not because the United States lacks infrastructure but because regulatory geography fragments access. Assisted dying is legal in 13 jurisdictions — a Score 3, not because the processes where it exists are poor, but because coverage is patchwork. Abortion access is similarly bifurcated, with nine states plus DC offering unrestricted access while others have effectively eliminated it. Psychedelic therapies are operational in Oregon (29 licensed service centers) and Colorado, with federal rescheduling moving fast; the infrastructure is real, but national scale is still two or three years away.

Wellness tourism categories — thermal balneotherapy, thalassotherapy, Ayurveda — score near the floor, not for lack of money or interest but because the United States never built the European-style medically integrated spa infrastructure. These are not the treatments patients cross the Atlantic to access.

Who the United States is for

The patient profile is specific: someone facing a diagnosis that is rare, complex, or poorly served in their home country; someone seeking access to a clinical trial; someone who needs a second opinion from a globally recognized center before committing to a treatment plan. The country's second-opinion infrastructure alone earns a perfect score, with dedicated international programs at every major academic medical center offering remote and in-person reviews within days.

For this patient, cost is secondary. The question is capability, and on that question, no other country consistently answers at the same level.
